- Only available in Minnesota
- You must be licensed for three years
or less
- You are currently a solo practitioner
- You are discipline-free
- No malpractice claims have been made against you
- You are not aware of any incidents that could reasonably result in a claim
- You have or intend to have acceptable Docket Control and Conflicts of Interest systems
- You meet nominal underwriting conditions
